**Job Summary:**
We are seeking a dedicated LPN (Licensed Practical Nurse) to join our team on a PRN (as-needed) basis to provide exceptional care to patients in Lorain County within a home health setting. The LPN will be responsible for administering medication, monitoring patient health, and collaborating with a multidisciplinary healthcare team to ensure the highest quality of care.

**Qualifications:**
- Current LPN license in the state of Ohio.
- Demonstrated experience in a home health or similar setting is preferred.
- Proficient in administering medications and providing direct patient care.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassionate and empathetic approach to patient care.

**Responsibilities:**
- Provide direct patient care in accordance with the established plan of care.
- Administer medications and treatments as prescribed by the physician.
- Monitor patient progress and report any changes to the healthcare team.
- Assist patients with activities of daily living.
- Educate patients and their families on health management and treatment plans.
- Maintain accurate and up-to-date patient documentation.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated patient care.

**Work Schedule:**
This position is on a PRN (as-needed) basis, with flexible scheduling based on patient needs and staff availability. Availability for weekend and evening shifts may be required.
