Job Summary:
We are seeking a Part-Time Physical Therapist to join our Home Health team on a PRN (as-needed) basis in Lorain County. The Physical Therapist will provide therapy services to patients in their homes, focusing on improving mobility, relieving pain, and restoring physical function.

Qualifications:
- Bachelor's or Master's degree in Physical Therapy from an accredited program
- Current state license to practice as a Physical Therapist
- Prior experience in home health or a similar setting is a plus
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively

Responsibilities:
- Conduct initial evaluations of patients' conditions and develop appropriate treatment plans
- Provide physical therapy interventions to address patients' needs and goals
- Educate patients and their families on home exercise programs and self-care techniques
- Collaborate with the healthcare team to ensure coordinated and effective patient care
- Maintain accurate documentation of patient progress and treatment outcomes
