Position Summary

We are seeking an experienced and compassionate Travel Registered Nurse (RN) to join our Home Health Care/Hospice team(s). The Travel RN will provide skilled nursing care to patients in their homes with assignments across Home Health/Hospice. This role is ideal for nurses who value flexibility, autonomy, and the opportunity to make a meaningful impact in patients’ lives.

Key Responsibilities

• Deliver direct patient care in the home setting, including assessments, wound care, IV therapy, medication administration, and disease management

. • Conduct comprehensive patient assessments, develop individualized care plans, and coordinate with physicians and interdisciplinary teams.

• Educate patients and families on care plans, medications, and self-management strategies to promote independence.

• Monitor patient progress, document care accurately, and communicate any changes in condition promptly.

• Ensure compliance with agency policies, state/federal regulations, and accreditation standards.

• Provide compassionate and professional care while traveling between patient homes within the assigned service area.

• Adapt quickly to new environments and maintain flexibility in varied clinical and cultural settings.

Qualifications

• Current and unrestricted Registered Nurse (RN) license in the state of OH

• Minimum 1–2 years of recent clinical nursing experience (home health or acute care preferred).

• Current BLS certification (ACLS preferred).

• Strong clinical assessment and critical thinking skills.

• Excellent communication, organizational, and time-management skills.

• Reliable transportation and a valid driver’s license.

• Willingness to travel and adapt to different geographic and patient population needs.
