Job Summary:
We are seeking a skilled Agency PRN Occupational Therapist to join our team to provide services in Home Health within Summit and Stark Counties. The Occupational Therapist will be responsible for assessing patients' conditions and needs, developing individualized treatment plans, and helping patients improve their ability to perform daily activities in their home environments.

Qualifications:
- Bachelor's or Master's degree in Occupational Therapy from an accredited program.
- Valid Occupational Therapist license in the state of practice.
- CPR certification.
- Previous experience in Home Health or a related setting is required.
- Strong communication and interpersonal skills.

Responsibilities:
- Evaluate patients' functional status and home environments to determine their occupational therapy needs.
- Develop and implement individualized treatment plans to help patients improve their ability to perform daily activities.
- Educate patients and their families on the use of adaptive equipment and techniques to facilitate independence.
- Work collaboratively with other healthcare professionals to ensure comprehensive care for patients.
- Maintain accurate and timely documentation of patient progress and treatment plans.

Other:
This position is PRN (as needed) and offers flexibility in scheduling. The Occupational Therapist will need to travel to patients' homes within Summit and Stark Counties, so reliable transportation is required. A passion for helping patients achieve their goals and improve their quality of life is essential for success in this role.
