**Job Summary:**
We are seeking a PRN Registered Nurse (RN) to join our team as an Agency Nurse for Home Health services in Lorain County. The PRN RN will provide quality care to patients in their homes, collaborating with the healthcare team to ensure optimal outcomes.

**Qualifications:**
- Valid RN license in the state of Ohio
- Minimum of 1 year of nursing experience, preferably in home health or a similar setting
- CPR certification
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Reliable transportation and valid driver's license

**Responsibilities:**
- Conduct initial assessments and develop individualized care plans
- Administer medications, treatments, and wound care as prescribed
- Monitor patient progress and update care plans as needed
- Educate patients and their families on proper care techniques
- Collaborate with physicians and other healthcare professionals to ensure coordinated care
- Maintain accurate and up-to-date patient records
- Adhere to all relevant policies, procedures, and regulations

**Schedule:**
This is a PRN (as-needed) position, offering flexibility in scheduling. The Agency PRN RN will work on a per-diem basis, filling in for regular staff as needed. Hours may vary based on patient needs and staffing requirements.
