Job Summary:
We are seeking a skilled Home Health Physical Therapist to join our team in Stark/Summit County. The Physical Therapist will be responsible for evaluating patients' conditions, developing treatment plans, and providing physical therapy services in patients' homes.

Qualifications:
- Bachelor's or Master's degree in Physical Therapy
- Valid state Physical Therapy license
- Proven experience in home health or a related setting
- Excellent communication and interpersonal skills
- Ability to work independently and manage a caseload effectively

Responsibilities:
- Conduct initial and ongoing assessments to determine patients' physical status and needs
- Develop individualized treatment plans based on assessment findings
- Provide physical therapy services to patients in their homes
- Educate patients and their families on exercises and techniques to improve mobility and function
- Document patient progress and communicate effectively with interdisciplinary team members
- Adhere to all regulatory and best practice standards in physical therapy
