Job Summary:
We are seeking a skilled Home Health Physical Therapist to join our team in Lorain County. The Physical Therapist will be responsible for evaluating patients' conditions, developing treatment plans, and providing physical therapy services in patients' homes.

Qualifications:
- Bachelor's or Master's degree in Physical Therapy
- Valid state Physical Therapy license
- Proven experience in home health or a related setting
- Excellent communication and interpersonal skills
- Ability to work independently and manage a caseload effectively

Responsibilities:
- Conduct initial and ongoing assessments to determine patients' physical status and needs
- Develop individualized treatment plans based on assessment findings
- Provide physical therapy services to patients in their homes
- Educate patients and their families on exercises and techniques to improve mobility and function
- Document patient progress and communicate effectively with interdisciplinary team members
- Adhere to all regulatory and best practice standards in physical therapy
