**Job Summary:**
We are seeking a dedicated Occupational Therapist to join our team in Lorain County, specializing in providing quality home health services. The Occupational Therapist will be responsible for assessing patients' needs, developing treatment plans, and implementing interventions to help them regain independence and achieve their goals within the comfort of their own homes.

**Qualifications:**
- Bachelor's or Master's degree in Occupational Therapy from an accredited program
- Valid Occupational Therapist license in the state of Ohio
- Previous experience in home health or a related setting is preferred
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Compassionate and patient-centered approach to care

**Responsibilities:**
- Conduct comprehensive assessments to identify patients' functional limitations and goals
- Develop individualized treatment plans based on assessment findings and in alignment with patient goals
- Provide occupational therapy services, including but not limited to: therapeutic exercises, manual therapy, and ADL training
- Educate patients and caregivers on home exercise programs and adaptive equipment as needed
- Document patient progress, interventions, and outcomes accurately and timely
- Collaborate with other healthcare professionals to ensure coordinated and holistic care delivery
