**Qualifications:**
- Bachelor's degree in Physical Therapy
- Valid state Physical Therapy license
- Proven experience working as a Physical Therapist, preferably in a home health setting

- Ability to create individualized treatment plans

**Responsibilities:**
- Conduct in-home physical therapy evaluations and assessments
- Develop personalized treatment plans based on assessments
- Provide education to patients and their families on at-home exercises and activities
- Monitor and report on patient progress
- Collaborate with other healthcare professionals to ensure comprehensive care
