Job Summary:

The primary responsibility of this role is to provide high-quality, personalized home health care services to patients within Lorain County. The ideal candidate will have a strong clinical background and a passion for delivering exceptional patient care in the comfort of their own homes.

Qualifications:
- Active Registered Nurse (RN) license in the state of Ohio
- Minimum of 2 years of nursing experience, preferably in home health or a similar setting
- Strong clinical skills and knowledge of best practices in home health care
- Excellent communication and interpersonal skills
- Ability to work independently and prioritize tasks effectively
- Reliable transportation and a valid driver's license

Responsibilities:
- Conduct initial assessments of patients' health status and develop personalized care plans
- Provide skilled nursing care, including medication administration, wound care, and patient education
- Monitor patients' progress and adjust care plans as needed
- Collaborate with a multidisciplinary team to ensure holistic care for patients
- Educate patients and their families on managing health conditions and promoting wellness
- Maintain accurate and up-to-date documentation of patient care activities
- Comply with all relevant regulations and standards of practice in home health care
